Scheduling an appointment with RHEMA Counseling Associates

Please complete the following RHEMA Biblical Counseling Forms and bring them with you at the date and time of your appointment. We encourage you to arrive at our office 15 minutes prior to the scheduled time of your appointment to allow our staff sufficient time to process your completed form. If for any reason beyond your control you are unable to bring the completed forms with you, please allow at least 30 minutes for processing.

You may receive a 20% discount on your first visit if

  •  your Biblical Counseling Forms are completed and returned 48 hours before your initial appointment, and
  •  you pay online for your first visit!

  • RHEMA Client Intake Forms
  • Your driver’s license
  • A valid credit/debit card
  • A list of allergies
  • A list of medications you are currently taking

RHEMA Counseling Associates Financial Responsibility Notice

Payment for services are due at the beginning of each appointment. We accept checks, cash, debit cards, MasterCard, Visa, and Discover. Any returned checks are subject to an additional collection fee of $25.00.

Rhema Counseling Associates No Show / Cancellation Policy

If you miss an appointment without canceling, or without a 24-hour notice, you will incur a fee of $35.00. You are asked to leave a signed check (with no date), payable to RHEMA, to cover this fee OR we will charge your credit card listed above. Your check will be held and used only to cover a No Show/Cancellation fee.
